Doctor’s physical form required for all campers. Please note: a printed copy of your child’s physical must be received by June 20, 2016. ** No faxed or emailed documents will be accepted** You do not have to use this specific form. Any standard Doctor’s Physical that includes vaccination records is acceptable (even if it has another camp’s name on it). The physical must be dated no more than three years before your child’s last day of camp.
Emergency Medication form MUST be completed and brought to camp with any medication (including epi-pens, inhalers, and benadryl). This form can be brought on your child’s first day of camp with your child’s medication.
